For all the SOAP note assignments, you will write a SOAP note about one of your patients and use the following acronym:

S = Subjective data: Patient’s Chief Complaint (CC).
= Objective data: Including client behavior, physical assessment, vital signs, and meds.
A = Assessment: Diagnosis of the patient’s condition. Include differential diagnosis.
P = Plan: Treatment, diagnostic testing, and follow up

P.J. is a 22-year-old African American male who presents in the facility with a chief complaint: “I am addicted to Oxycodone’’.

History of Present Illness/ Presenting Problem

The client seen at the facility with oxycodone addiction noted that he started using it as a recreational drug last year during the summer. The child first accessed the drug through his friend. He did not realize that the more he used it, the more addicted he became. He narrates that in the initial stages, he snuffed half the pill once a month and slowly increased the frequency of usage to seven times a month. P.J. further states that since he last took the drug, he has not been able to get real sleep. The client’s family is concerned about the addiction and is seeking help to manage the dependence. APA
