In the recent practicum, my preceptor and I experienced a unique case. The case involved a 10-year-old female, N.K who has been a victim of childhood negligence and abuse. The Pt was rescued after attempting suicide by throwing herself on a busy highway. During the assessment, N.K expressed her frustrations to my preceptor and narrated the traumatizing experiences she has experienced since she was four years. She painfully narrated that at the age of 4, she was sexually abused by her mother’s boyfriend, who has bipolar disorder. The patient recounts numerous occasions when the man physically abused her and her mother. The patient is now in the custody of her grandmother, has poor academic performance. Additionally, she expresses her disappointment in her mother who should have protected her from being abused.

The Pt had some scars on her body and explains how she used a knife to cut herself. Lately, she explained that she has been feeling hopeless, easily irritated, having difficulty falling and staying asleep. The patient also constantly experiences suicidal thoughts. She does not want to interact with other people, has difficulty concentrating, and eats less than usual. The patient says that she lost the purpose of living when she experienced abuse from a man who lived with her mother. She does not ever want to see her mother since she did not protect her from the abuse. She adds that if she had access to a gun, she would shoot the man who defiled her.
After the interview, my preceptor conducted a physical examination to help rule out other health problems. Also, a psychiatric evaluation and carefully examination were performed and it was concluded that the child most likely has PTSD. My preceptor prescribed Fluoxetine 10mg to be taken by mouth once per day, Sertraline 25 mg to be taken twice daily, and Vilazodone 10 mg orally once a day.
