Despite evolving healthcare needs, laws and regulations governing scope of practice, licensure, and staffing are often outdated. Political resistance and fragmented state regulations hinder nationwide progress, especially for advanced practice registered nurses (APRNs) who face varying levels of autonomy. The U.S. continues to face nursing shortages, particularly in rural and underserved areas, due to inadequate educational capacity (e.g., lack of faculty and funding). An aging workforce and increasing patient complexity only exacerbate these shortages.
Issues in Today’s Workforce Culture
Many healthcare institutions are resistant to change due to entrenched hierarchical systems, lack of innovation in staffing models, and concerns about costs. Nurses often face burnout, moral distress, and unsafe working conditions (e.g., mandatory overtime, understaffing), yet these concerns are not always prioritized by leadership. While nurses are the largest group of healthcare professionals, they remain underrepresented in key decision-making roles and policy forums. Without strong advocacy, workplace issues like violence prevention, fair staffing ratios, and professional autonomy remain inadequately addressed.
Healthcare reforms (e.g., the ACA) have increased demand for care, especially primary care, without proportionate investment in workforce infrastructure. Cost-cutting measures by hospitals and systems often result in understaffing and reduced resources, negatively impacting workforce well-being. The persistence of these issues reflects a combination of regulatory inertia, underinvestment, lack of political will, and systemic undervaluation of the nursing workforce.
